Gift Communication Form

New Donor Agreement Request Form

New Donor Agreement Request Form

Once gift negotiation with donors, deans, faculty, UA leadership and/or other relevant parties is complete with final terms of an agreement reached, development officers will initiate the drafting and signature routing of the final agreement by completing and submitting this simple request form to the CSU Foundation.

Standard Language Provisions in a Fund Agreement

This resource is available to share only if the donor wants to see an example of the standard language found in a fund agreement, it is not meant to be included as part of a proposal the donor is reviewing. The black font denotes language approved by CSUF and/or rooted in its board of directors’ policies, so unfortunately that text cannot be modified.

Selection Criteria Options

This resource provides a menu of criteria options a development officer could share with donors planning to fund a scholarship at the Presidential level and above. Please see the Scholarship Criteria Language Guide resource for examples of how the criteria will be worded in the final fund agreement.

Scholarship Criteria Language Guide

This guide further defines the scholarship criteria outlined in the purpose section of the agreement request form. The intent of this document is to be a resource for development officers working with donors who are considering establishing a scholarship. The approved language could be used within proposals, so donors have the opportunity to preview the wording of scholarship criteria to ensure it aligns with their intent.

Quasi Endowment Request

These forms are completed in order for a Unit/College (modifying anything in capital letters) to request a quasi. If you’d like this request to be considered by the CSUF board, please secure your dean’s signature and return it via email no later than 2 weeks prior to the next Foundation board meeting to assure it’s included in the board meeting materials. If approved, your gift agreement specialist will promptly provide the agreement draft for signature.
